In a fierce competition, the team finished behind the United States and Australia, with a time of 3'10''35, narrowly beating China.
The race saw a strategic change in the lineup, with debutant Paolo Conte Bonin stepping in after Lorenzo Zazzeri's underwhelming start.
Meanwhile, Sara Curtis made a remarkable Olympic debut in the women's 4x100 freestyle relay, helping Italy qualify for the final with a time of 3'36''28, just shy of the national record.
The Italian team, which includes Curtis, Sofia Morini, Chiara Tarantino, and Emma Virginia Menicucci, will compete in the final later tonight, aiming for a podium finish.
